A.2.1.2.2 Test number format.
a. Test numbers shall contain nine digits exclusive of dashes, change, and revision characters. Test numbers shall be structured in accordance with the format depicted on figure A-1.

b. The first five digits of the test number shall be based on ESWBS and shall comply with the master HSC
cross reference (see A.2.1.b).
c. The sixth digit of the test number, the test stage, shall be determined and assigned based on the definitions in section A.3.
d. The last three digits of the test number shall be a serial number which is assigned to the respective ESWBS and test stage combination. The serial number digits shall be sequentially assigned based on review from TEAMIS Repository for the next available number, and shall not represent prerequisite testing relationships within any given test package.
e. Revisions and changes: Test numbers for existing test procedures may be modified by revisions (REV) or changes (CHG), if those changes to the test procedure (TP) have been formally approved by the test problem report (TPR) or test change proposal (TCP) process. Any such TP changes must be documented in the REVISION RECORD of the TP. TP changes that affect half or more of the TP pages or more are defined as Revisions; TP changes that affect less than half of the pages are a change (CHG). For example, the AN/SPS-49A(V)1 Initial
Light Off test is TP number 45249-3-030. If the TP is updated in response to a TPR that affects more than half of the pages, the updated version of the TP would be 45249-3-030 REV A. If the revised version was then used on another ship and another TPR was submitted that required changes on less than half of the pages, the revised TP would then be 45249-3-030 REV A CHG 1.
A.2.1.2.3 Test number determination and request.
a. After development of the test document has begun and test stage determination has been completed, the
Test Number/Test Title Request Form (see figure A-2) shall be initiated by the test development organization.
b. Responsible personnel within the test development organization shall determine the first six digits (ESWBS and test stage) of the technically correct test number. It shall be the responsibility of the TDA to determine a proposed test number based on that organization's knowledge of ESWBS, and on the test engineering performed to determine the scope of required testing and the appropriate test staging.
A.2.1.2.4 Test number assignment.
a. A single test number shall be assigned and issued for each test document related to a specific test requirement. The assigned test number shall be affixed to the test document.
b. The assigned test number shall be retained with the test document through each phase of development and throughout its life cycle.
c. After a test number has been assigned and issued, it shall not be used or re-used for any purpose other than that intended by this test numbering process.
