The COMMENTS sheet is used by the test conductor and Government witness to record significant events, conditions, and observations pertinent to test conduct.

This section is not applicable to appendices or addendums.

The COMMENTS sheet will be used to indicate the numbers of any Test Problem Report (TPR), Software Trouble Report (STR) or Computer Program Problem Report (CPPR) and is prepared during the conduct of the test.

A line for recording equipment/system identification or serial number shall be included at the beginning of this section.  Prerequisites as listed in GENERAL INFORMATION section 6 shall be listed here also with a signature block to verify completion of the prerequisites.
