A.4.2.3  Test title determination and request.

a.    The responsible personnel within the test development organization shall determine a proposed technically correct test title in accordance with this procedure.  The recommended test title shall be requested, via Test Number/Test Title Request Form (figure A-2), concurrently with the test document number.

b.    Once a test title has been assigned and entered into the TEAMIS Repository, it shall not be changed unless a TCP is prepared and submitted with appropriate approval signatures.

c.    The assigned test title shall be affixed to the test document in accordance with the format requirements of

Appendix B.
